From Intake Form to Follow-Up

Most people hesitate before booking because they do not know what happens after they press submit. This page walks through the whole sequence, so you can decide with the facts in hand.

What actually happens after you submit

A consultation is not one event. It is a form, a match, a payment step, a session, and a plan you can actually use on a Tuesday night. Here is the sequence, in the order you meet it.

Nothing here is a commitment. Reading it through and deciding a session is not right for you yet is a perfectly good outcome.

  1. Fill the intake form About ten minutes of plain questions about your goal, your usual day of eating, sleep, and current medication or supplements.
  2. Review and matching A coordinator reads your answers within one working day and routes you to the dietitian whose work fits your goal.
  3. Scheduling and payment You pick a slot in WIB, WITA or WIT, pay by invoice or bank transfer, and get a reminder the day before.
  4. Your first session About 45 minutes by video call or chat, working through your form and shaping a plan around your real meals.
  5. Plan and follow-up Written notes land after the call, and shorter follow-ups keep the plan alive as your weeks change.

Step one: the intake form

Rough is fine. A gesture at yesterday beats a precise guess at numbers.

The form takes around ten minutes. It asks for your goal, a rough picture of a normal day of eating, activity level, sleep pattern, relevant medical history you are willing to share, and any supplements or medicines you currently take.

Nothing needs to be precise. Two plates of rice at dinner most days is a better answer than a guess at grams, and it tells your dietitian more about your actual week. If a question does not apply, say so in your own words rather than scrolling past it. The form is written in plain English and you can answer in the order you like.

  • Your goal, in whatever words you use for it
  • A sketch of a normal day of eating
  • Activity level and how you sleep
  • Supplements and medicines you currently take

Step two: review and matching

A coordinator reads your submission within one working day. If your goal is sports nutrition, you go to a dietitian who works with training schedules. If it is pregnancy or child feeding, you go to someone in life-cycle nutrition. You receive your dietitian's name and a short note on why they were matched to you.

Matching is not a queue. Two people with the same goal can land with different dietitians because the second set of answers — sleep, medicines, the shape of your working week — often decides who is the better fit. If you already know who you want to speak to, say so in the form and the coordinator will work with that.

Step three: scheduling and payment

You choose a slot that fits your timezone, which for most clients in Indonesia means WIB, WITA, or WIT. You receive an invoice or bank transfer instruction before the session; if you are near Yogyakarta, you can also pay in person. We confirm once payment clears, and you get a reminder the day before. Sessions are not confirmed without it.

Choosing a slot

Evening and weekend slots exist because most clients work. Tell the coordinator your timezone once and every later booking will be shown to you in it.

Paying before the session

Bank transfer is the usual route, with an invoice sent to your email and phone. In-person payment is possible in Yogyakarta by arrangement, and confirmation follows as soon as the payment clears.

Reminders and rescheduling

A reminder arrives the day before. If something changes, tell us as early as you can and the booking moves without a second charge.

Prices and payment instructions are set at the time you book, so nothing here needs to be decided before you have a slot in front of you.

What happens during the session

A first session runs about 45 minutes by video call, or across a chat window if you chose that format. Your dietitian works through your intake form with you, asks follow-up questions, and starts shaping a plan around your actual meals. You are encouraged to interrupt.

The output is written notes and a personalized plan with portions, swaps, and a realistic starting point for the coming two weeks. Nothing is filmed or recorded unless you ask, and the notes are sent to the same email you booked with.

Before the call

Bring your notes on the past day of eating, whatever medicines or supplements you take, and the questions you have been carrying. There is no need to weigh anything beforehand, and no fitness tracker is required. If you would rather type than talk, choose the chat format when you book and the same 45 minutes run in the message window instead.

The plan you receive

Plans are written in English and use Indonesian food names, so you read one palm-sized piece of grilled fish with a bowl of rice and stir-fried kangkung rather than an abstract exchange list. You get a few days of example structure, not a seven-day menu you must follow exactly. Flexibility is built in because no two weeks look alike.

The plan also lists the swaps that keep it workable: what to do when the office orders nasi padang, how to handle a week of night shifts, what to reach for when you are travelling and the only option is a roadside warung. Every plan includes the portion sizes in household measures, not only in grams.

What is inside a written plan

  • Portions you can measure with your hands

Palm, cupped hand, thumb, and bowl sizes, alongside the gram figure for anyone who prefers it.

  • Swaps for the meals you actually eat

Rice, noodles, tempeh, tofu, eggs, and the usual market vegetables, with alternatives when one is unavailable.

  • A realistic starting point for two weeks

Three or four days of example structure rather than a rigid week, so the plan survives an unexpected schedule.

  • Notes from the conversation

The reasoning behind each change, so a follow-up session starts from why, not from scratch.

Chat and WhatsApp coordination

Many people in Indonesia prefer to ask a quick question by WhatsApp before committing to a session. We use it for coordination: slot changes, payment confirmation, and clarifying what the intake form asked.

Nutrition advice itself happens in the scheduled session so there is a record of what was recommended. That record matters later, when you want to know why a plan changed.

You can reach the coordination desk during working hours, Monday to Friday, 09:00–18:00, at the studio line on +62 274 589 321.

Follow-up sessions and coaching format

One session rarely changes a habit. Follow-up sessions run shorter, usually 20 to 30 minutes, and focus on what actually happened since you last spoke. Some clients choose an ongoing coaching rhythm of regular check-ins over several months, which suits people who want accountability rather than a one-off plan. You decide the cadence; nothing renews automatically.

A single follow-up

One 20–30 minute session a few weeks later, useful after a trip, a busy stretch, or a plan that needs adjusting.

An ongoing rhythm

Regular check-ins across several months, scheduled as you go, for people who do better with a standing appointment.

Follow-ups work best when you note what happened in the days between sessions — what you ate, what you skipped, what got in the way. A phone note is enough. Your dietitian will read it and adjust accordingly.

When we suggest a different route

Some situations belong in a hospital or with your physician: disordered eating, unmanaged diabetes, kidney disease, and anything requiring clinical monitoring. We also decline cases where a client wants a rapid body-change plan we cannot support responsibly. In those cases we explain the reason and suggest the appropriate setting.

This is not a sales page, so it is worth being plain: a consultation here is dietary guidance and coaching. It does not replace medical care, and it does not diagnose or treat a condition. If you are already under a doctor's care, tell us in the form and the dietitian will work with what your physician has set out rather than around it.

Knowing where a service stops is part of using it well. If your situation sits outside what we can responsibly handle, we will say so early rather than take a booking we should not.

What you need before the first session

A stable internet connection, a quiet half hour, and your notes on the past day of eating. If you take medication or supplements, have the names available. You do not need a scale, a fitness tracker, or lab results to begin. Bring questions; the session is more useful when you ask them early.

Bring

Rough notes on yesterday's meals, your activity level, and any supplements or medicines by name.

Skip

Weighing food, counting calories, or buying equipment before the call. None of it changes the first session.

Arrange

A quiet half hour and a stable connection. A phone with headphones works as well as a laptop.

Questions before you book

Short answers to the three things people ask most. If yours is not here, ask it directly and we will answer it plainly.

Ask a question first

How long does it take to get a session after submitting the form?

Your form is reviewed within one working day, and most clients book a slot within the same week. Slots depend on dietitian availability and your timezone.

Can I reschedule?

Yes, with notice before the session. Tell us by phone or through the contact page as early as you can, and we move the booking without charging a second time.

Do I need to prepare anything?

Rough notes on yesterday's meals, your activity level, and any supplements or medicines. No weighing or calorie counting beforehand.

Ready when your week allows

Submit the intake form from the contact page whenever you are ready. If you would rather ask something first, that route is open too and costs you nothing but the question.
